GUARDIANSHIP ANNUAL REPORT

Common mistakes

  1. Inadequate Consultation with the Ward: Many guardians fail to involve the ward in discussions about their needs and preferences. This oversight can lead to incomplete or inaccurate information on the form, which may affect the court's understanding of the ward's situation.

  2. Incorrectly Filling Out Financial Information: Some guardians mistakenly fill out the financial sections even if they are partial guardians without conservator powers. This can lead to unnecessary complications and delays in processing the form.

  3. Neglecting to Update Contact Information: Guardians often forget to check the box indicating a change of mailing address. If the court cannot reach the guardian due to outdated contact details, it can create significant issues in communication.

  4. Failure to Sign Under Oath: A common mistake is not signing the form in the presence of a notary public or court clerk. Without this signature, the form is not valid, and the guardian may face legal repercussions.

Detailed Steps for Filling Out Alaska Guardianship

Completing the Alaska Guardianship form requires careful attention to detail and an understanding of the ward's current situation. This process helps ensure that the court receives a comprehensive view of the ward's needs and circumstances over the past year. Follow these steps to fill out the form correctly.

  1. Begin by entering the case number and the name of the ward at the top of the form.
  2. Fill in the ward's date of birth and residential location.
  3. Provide the ward’s telephone number.
  4. Indicate the reporting period by entering the start and end dates.
  5. Enter your name, daytime phone number, and mailing address. If this address is new, check the appropriate box.
  6. Fill in your residence address and indicate if you live with the ward. If so, specify your relationship to the ward.
  7. List the areas in which you have decision-making authority for the ward, such as housing, medical care, and financial management.
  8. If applicable, indicate whether a separate conservator has been appointed for the ward and provide their name.
  9. If you charge fees as a private guardian, confirm whether there is a court order authorizing payment of those fees.
  10. Answer whether there is a current need for change in the guardianship and provide details if necessary.
  11. Provide information about the ward's housing, including the name and address of the facility or residence, and answer questions regarding any recent moves.
  12. Detail the ward's medical care, including the names of medical professionals seen in the past year and any medical problems being addressed.
  13. Describe the ward's educational or job training status, including any discussions held with the ward about their needs.
  14. Provide information about the ward's employment status and whether it would be beneficial for them to seek employment.
  15. Outline the ward's social and recreational activities and any plans to enhance these opportunities.
  16. Indicate how often you have visited the ward in the past year and any other forms of contact.
  17. Once all sections are completed, sign the report under oath in front of a notary public or court clerk.

After filling out the form, it is essential to ensure that all information is accurate and complete. This report will be submitted to the court, and it is crucial that it reflects the ward's current situation and needs effectively.
